Abstract
Introduction: Tension-type headache (TTH) is the most common primary headache disorder
with an estimated annual prevalence of 26% to 38%. The causes of the pathophysiology are still
insufficiently understood, and an adequate therapy for chronic tension-type headache (CTTH) is
difficult to find. Since the pathophysiology is still largely unclear, the aim of this study was to
search for microstructural changes in CTTH using Diffusions tensor imaging (DTI) in order to
better understand the pathogenesis.
Material and Methods: We included a total of 9 female patients with CTTH aged 18 to 63
years (mean age 37.9 +/- 18.6 years) and 10 healthy female control subjects aged 19 to 62 years
(mean age 37.3 +/- 14.3 years). The headache patients were thoroughly questioned regarding
inclusion and exclusion criteria in a preliminary interview and examined by a neurologist expe-
rienced in headaches. All participants underwent magnetic resonance imaging (MRI) with diffu-
sion tensor imaging (DTI) sequence. From the diffusion tensor, fractional anisotropy (FA), axial
diffusivity (AD), radial diffusivity (RD), NODDI parameters, orientation dispersion index
(ODI), and neurite density index (NDI) were calculated.
Results: Using the various scalar indices, we were able to identify microstructural changes in
the right cerebellum, right tectum, right occipital fusiform gyrus, left cerebral cortex, left inferi-
or frontal gyrus, right medial temporal gyrus, left medial temporal gyrus, right medial frontal
gyrus, right angular gyrus, right postcentral gyrus, left superior longitudinal fasciculus, right
fusiform/temporal parahippocampal gyrus, and right cingulate gyrus.
Conclusions: Microstructural alterations were found in several brain areas in patients suffering
from CTTH. These are associated with brain areas that belong to the pain matrix, the trigeminal
system or association fibers and interact.
