Psychiatrisch-psychotherapeutische Behandlung von Müttern mit Kindern im Vorschulalter: Zusammenhänge zwischen Funktions- und Beziehungsaspekten

Abstract

The study conducted here aimed to establish correlations between relationship parameters and functional aspects of the patient and the child in mentally ill female patients with a child aged 2 to 6 years and to investigate the effectiveness of mother-child therapy by looking at functional levels. For this purpose, various questionnaires of 91 mother-child dyades were examined between 2015 and 2020 at the Psychiatric Center North Baden in Wiesloch and a telephone follow-up sur- vey was conducted with 20 patients. A further focus was placed on the connection of the factors resilience, self-management and satisfaction with living conditions. In the main studies, linear regression was used to show that a high level of functioning in the mother-child relationship at admission and low infant symptomatology at admission were predictors of high treatment success. A strong treatment outcome, a high psychological, social, and occupational functioning level of the patient at admission, and a strong improvement in this functioning level predicted a high functioning level of the patient at discharge. With the help of correlations, a relationship was established between a high level of functioning in the mother-child relationship and low child problem behavior, functional parenting parameters, and strong maternal care. The ancillary studies were able to support the effectiveness of mother-child therapy. Using regression analyses, it was possible to show that high resilience is a predictor of both good patient self-management and high satisfaction with living conditions. Furthermore, correlations of resilience with both self-management and satisfaction with living conditions could be found. Also, an increase in the patients' psychological, social and occupational level of functioning could be shown in the course of therapy. Based on the evaluation of satisfaction scales in the catamnesis, first indications for a long-term effectiveness of the therapy could be recorded. A high functional level of the patients at discharge was also shown to be a predictor for a high satisfaction score at catamnesis. The recorded results of the main and secondary investigations can be well classified in the research results of comparable studies. Nevertheless, these studies mostly concern the patient group of mothers with children in infancy, whereas the 90 patient group of mothers with children aged 2 to 6 years investigated here is largely unexplored. Thus, for this patient group, correlations between relationship parameters and aspects of functioning could be established and the efficacy of mother-child therapy could be demonstrated, providing a good basis for further research on mothers with preschool-aged children. In general, it is clear that there is both a need for mother-child therapy for patients with children 2 to 6 years of age and a need for research on this patient group.
