Abstract
The present study was carried out as a retrospective evaluation of 219 performed dacryocystographies and 145 balloon dacryocystoplasties with long-term results in the follow-up. All patients presented to our clinic with persistent epiphora. All patients received lacrimal syringing and digital subtraction dacryocystography.
The aim of this study was to investigate whether sufficient information about the cause and location of the tear duct obstruction can be obtained with dacryocystography. Another goal was to use empirical evidence to identify criteria that ensure the best clinical success of a subsequent balloon dacryocystoplasty in order to be able to determine more precisely in the future for which patient collective the balloon dacryocystoplasty promises the best clinical success.
Using dacryocystography, the location and cause of the obstruction could easily be determined. In addition, the dacryocystography provided additional information about the cause of the obstruction such as polyps or dacryolites.
Our results show that balloon dacryocystoplasty can achieve success rates of over 80% for obstructions of the nasolacrimal duct. However, the success depends on the type of obstruction (stenosis or occlusion) and on the position of the obstruction in the nasolacrimal duct. Balloon dacryocystoplasty for presaccal and combined presaccal / postsaccal obstruction achieves significantly lower success rates of up to 60%. In the case of polypoid obstruction of the lacrimal duct, balloon dacryocystoplasty achieved a success rate of only 22%. If polyps are detected, we recommend performing balloon dacryocystoplasty only in multimorbid patients or if there is a contraindication for an operation or at the patient's request.
Overall, balloon dacryocystoplasty, as evidenced by our data, proves to be a successful and low-complication interventional radiological procedure, which is a minimally invasive alternative to surgery and should be offered especially in narbitic obstruction of the nasolacrimal duct and for presaccal obstruction.
