Abstract

This theses deals with the status and the problems of a 3rd world hospital in treating patients. The research took place at the Kenyatta National Hospital in Nairobi, Kenya. 75.6 % of the patients were male. The average age of all patients was 48 years. 44.7 % of the tumors were located in the epipharynx, 25.1 % in the larynx, 7.5% in the hypopharynx, 5.6 % were located in the oropharynx and 5.4 % in the esophagus. At all 11.7 % of the 443 patients suffered from an otherwise situated malignoma. In 75 % the malignoma was only diagnosed with the size of T3 and T4. At 23.9 % of the patients the tumor size was given with T1 or T2. The symptoms already existed at an average of 26 weeks before first presentation. 13 Days were needed for the exact diagnosis. A Biopsy and histopathological examination were undertaken in 81,9 % of the patients to verify the diagnosis. 11.1 % of the patients were diagnosed only by inspection and examination. 53 % of the patients suffered from a squamous cell carcinoma, 39.5 % had lymphoepithelial carcinoma as histological diagnosis. 27.1 % of the patients had no treatment, 79.9 % of the treated patients had radiotherapy, 11.1 % had an operation.
