Abstract
Submandibular glands pathologies present as submandibular swellings usually associated with pain during meals. Sialadenitis, usually accompanied by sialolithiasis is the most common pathological entity leading to the extirpation of the submandibular gland. Gold standard of therapy is the transcervical extirpation of the diseased gland. The aim of this doctoral thesis is to retrospectively analyse 87 patients who were treated in the Department of Otolaryngology, Head & Neck Surgery, University of Marburg, Germany, between 2001 and 2010 for submandibular gland pathologies. The present thesis focused on the intraoperative and postoperative complications resulting from this operation. The results of this thesis are discussed reflecting the relevant literature.
The average patient’s age in the present study was 52.7. Fifty eight percent of our patients were male, while forty two percent were female. Most common pathologies that led to operation were sialolithiasis (73.5 %), benign tumors (18.5 %) and malignant tumors (8 %). The most common symptoms were a submandibular swelling (100 %), pain (64 %) and pus secretion from the Wharthon canal (13.8 %). Ultrasound examination was performed in all cases (100 %), but MRT was also often used for tumor identification (31 %). In our study Computertomography was rarely used (4.6 %).
Therapy of choice is the transcervical extirpation of the gland mainly due to the easy access, the low complications rate and the short operation time. Complications include wound healing disruption in form of hematoma and seroma and nerve palsies of the facial, the lingual and the hypoglossal nerve. Three patients developed a hematoma and one a seroma. Five patients presented a temporary palsy of the facial (5.7 %) and the lingual nerve (5.7 %), while only one case of transient palsy of the hypoglossal nerve was observed. The present study shows that the transcervical extirpation of the submandibular gland is an easy, fast and safe procedure with low complications rate, leading to minimal aesthetic damage
