Abstract

Objectives The success of a root canal treatment is not always guaranteed. There are many factors that require an endodontic retreatment. According to the current state of science, orthograde retreatment is the treat of choice in case of endodontic fail-ure. The removal of the root filling material plays a decisive role in the quality of the subsequent re-obturation. The aim of this study was to find a minimally inva-sive retreatment method that allows for a complete removal of the root filling ma-terial without necessitating weakening the dentin by increasing the canal diameter. Therefore the use of the F360 instrumentation system used as a retreatment sys-tem, the benefits of using eucalyptol in a second retreatment step, and the activa-tion of the oil using 3 different techniques should be investigated. Methods 60 roots of human teeth with a round root canal smaller than ISO 45 were short-ened to 14 mm and processed to a working length of 13 mm using the F360 sys-tem up to .04/# 45. Then the roots were sectioned longitudinally in mesio-distal direction and repositioned, followed by obturation using a gutta-percha cone #45 and sealer AH Plus in a single-cone technique. The roots were divided into 4 groups (n=15) for further investigation. After 3 weeks storage under clinical condi-tions ( 37°C, 100% humidity), the retreatment of the root canal fillings was carried out using the F360 file #45 and the root canal was divided into coronal, middle and apical areas of 3 mm spacing. The residual obturation material was analyzed for each root half per canal section using digital microscope Keyence VHX 5000 at 200-fold magnification. Following renewed reduction, eucalyptol was added as part of the 2nd retreament step to the canal for 2 min and activated for 1 minute using (2) the ultrasound operated IRRI S tip, (3) the mechanically driven Canal-Brush, (4) the sonically activated SF65 or (1) an additional contact time of 1 min without activation as control group. Concluding, a further quantitative assessment of the residual obturation material was performed under the digital microscope with subsequent statistical analysis. Results The second retreatment step resulted in improved cleaning efficiency in all four groups. Significant differences regarding the efficiency of activation techniques could were exclusively within the apical canal section. Group 4 (SF65) achieved a significantly increased retreatment efficiency of additional 25.50% versus 5% (Mann-Whitney test: p=0.001*) in group 3 (CanalBrush) and 9.50% (Mann-Whitney test: p=0.005*) in group 1 (no activation). Significant differences were only found in the root canals of group 4. In the apical section 25.5% of the obtura-tion material could be removed, compared to 14% (Wilcoxon test: p=0.018*) in the coronal and 16% (Wilcoxon test: p=0.020*) in the middle section. Conclusion None of the investigated activation techniques resulted in acomplete removal of the root filling material. Compared to established instrument systems the applica-tion of the F360 system led to satisfactory results in terms of retreatment efficien-cy. However, an additional retreatment technique is recommended. The use of eucalyptol in a 2nd revision step is recommended. Particularly, the sonic activation of the solvent achieved a significant reduction of the root filling material in the api-cal canal section. Based on this study, the additional sonic activation of eucalyptol provided a favorable minimally invasive alternative to the established root canal enlargement. Further studies are required in order to decide which retreatment technique may be universally recommended.
