Abstract

Background and aim of this study Sufficient polymerization is an important factor for durability of restorative therapy with light-curing resin composites. Using a conventional polymerization device, minimum expo-sure time is 20 s. Innovative technologies promise high-intensity lightcuring in 3 s, being attractive from a clinical point of view due to their time savings in treatment. This study compares the effect of high-intensity (3 s with 3,500 mW/cm²) and conventional (20 s with 1,480 mW/cm²) lightcuring on bonding performance, considering different incre-ment thicknesses, and conditioning methods. A conventional nano-hybrid and bulk-fill resin composite were compared to an ultra-short-term polymerizable resin having been specifi-cally designed for high-intensity curing. Material and methods The study was accomplished in vitro by use of 50 human third molars that were free of car-ies and enamel or dentin abnormalities and extracted for therapeutic reasons. A cavity with the dimensions 4 mm x 4 mm x 4 mm was prepared in all teeth. The teeth were divided into eight experimental groups (n=5) and etched depending on their group classification with the etch-rinse or selective-enamel-etch method and bonded with a light-curing universal adhe-sive AdhesSE Universal, which is part of the 3s-PowerCure product system (Ivoclar Vivcadent, Schaan, Lichtenstein). Four groups were filled with light-curing high-viscosity and flowable bulk-fill composites of the same product system and cured with the associated high-performance 3 s PowerCure Bluephase polymerisation lamp in 3s-PowerCure mode. Another four groups were filled with a conventional nano-hybrid composite Venus Pearl (Heraeus Kulzer GmbH & Co. KG, Hanau, Germany) or the bulk-fill-flowable SDR (Densply Sirona, Kon-stanz, Germany) and cured with a conventional Elipar LED curing light (3M ESPE, Seefeld, Germany) for 20 s. Dentin bonding performance was then measured using the microtensiles methods. Results and observations In all groups, incremental layering significantly increased dentin adhesion (ANOVA: F(1.81) = 7.45, p = 0.008, ƞ2 = 0.08). When using ultra-fast curing and high-intensity curing equip-ment, self-etch conditioning resulted in significantly fewer pre-test failures. (Logistic regres-sion: B = 0.50, SE = 0.15, Wald = 11.77, p < 0.001, OR = 1.64) No significant differences in dentin adhesion were found for the two compared polymerization systems (ANOVA: F(1.81) = 0.57, p = 0.454, ƞ2 = 0.01). Practical conclusions It was shown that layering still reveled to be beneficial even for ultra-short-time polymeriza-tion using the bulk-fill technique. In the case of enamel conditioning, on the other hand, enamel etching using the selective-enamel-etch method should be preferred. In Class-I cavi-ties, the tested ultra-short-term polymer achieved dentin adhesion values comparable to those of the standard resin composites.
