Abstract

Introduction: The cementation of tibial implants during total knee arthroplasty is a gold-standard ensuring proper joint flexibility. In contrast, due to its limited use, only sparse data exists to date regarding unicondylar arthroplasty. In this study we compare cemented with cementless unicondylar knee arthroplasty and aim to define both clinical and radiological differences in treatment outcome. Materials and Methods: 106 patients were included in this retrospective study who had received a medial unicondylar prostheses from one surgeon. Of these, 7 patients had died 42 patients (median age 81±7 years) who received a cemented and 64 (median age 73±7 years) a cementless knee arthroplasty using similar surgical techniques. Well-established clinical (VAS, HSS, KSS, UCLA, WOMAC) and life quality (SF-36) criteria were used to evaluate treatment outcome and radiographic results. Ewald’s criteria of periprosthetic loosening zones were used to evaluate the x-rays. Results: The cementless patient group presented with better clinical scores (HSS, KSS, UCLA, WOMAC) except in the Pain Analogue Scale. Also life quality was significantly better in the cementless group. Only with regard to physical function and social role did the cementless group approximate to the normal collective. Moreover, radiographic analysis revealed more and larger periprosthetic loosening zones in tibial zone 2 in the a.p. x-ray in the group of cementless implants. Conclusion: The inferior clinical results characterising the cemented arm were probably due to the higher average age of patients in this group. Regarding the radiological loosening zones, we did not detect any differences in the techniques of arthroplasty, although physical activity and mechanical mobility were higher in the cementless group.
