Psychoonkologisches Screening zur Identifikation und Evaluation belasteter Patienten in der ambulanten uroonkologischen Therapie

Abstract

In the past years, the role of psychooncology in the treatment of carcinoma patients has increased and is now part of the guidelines for cancer therapy. Studies showed the impact of an oncological disease on psychological health and the development of distress on the one hand and the positive influence of psychological treatment in such a situation on the other hand. The knowledge about distress in an ambulant setting of urooncological treatment and therapy is still not satisfying. To investigate the primary question of a possible psychooncological undersupply during an ambulant oncological therapy we used two screening tools (FBK10 and Distress Thermometer). The second aim was to evaluate the patients’ self-assessment in terms of the need for psychological support. Our study included 95 patients undergoing an ambulant therapy for prostate-, kidney-, bladder- and germ cell cancer at the University Hospital Marburg in a cross-sectional study. After drop-outs we obtained 80 correct records of the FBK-10 (cut-off ≥ 15 points) and 82 records of the Distress Thermometer (cut-off ≥ 5 points). We used SPSS to analyse the collected data. It showed an underrepresentation of female sex which leads to the fact that we cannot make a strong statement regarding female patients in an ambulant urooncological setting. 21,7 % / 37 % (FBK-10 / DT) of the patients with a PCA reached the cut off. With RCC 56,8 % / 41,2 %, with bladder cancer 33,3 % / 66,7 % and with germ cell cancer 28,6 % / 50 % reached the cut-off-levels. Both screening tools showed high rates of patients suffering from psychological distress which makes an undersupply of psychooncological trace therapy apparent. In contrast to the high scores within the FBK-10 and DT, only 8,7 % of the patients that responded to the question of a psychooncological therapy, expressed the wish to receive such a treatment. Reasons for this high rate of declining a psychooncological therapy can have many reasons, e.g. patients’ wrong self-assessment or fear of stigmatization. These low levels of patients asking for a therapy should not lead to underestimate the results of the used screening tools. The results underline the mismatch between the knowledge of a strong psychological stress and high screenings results in all the analyzed groups on the one hand, and the rejection of a possible additional therapy by the patients on the other. These are references for a still not well-integrated and accepted therapy concept of psychooncological treatment for patients with cancer. Therefore, the integration and information about trace therapy options and their importance within a cancer treatment should be improved.
