Abstract

The hypercapnic chemosensitivity (HCVR) seems to play a decisive role in the genesis of Cheyne-Stokes respiration in heart failure patients. This type of respiration occurs mainly at older ages and the male gender are more vulnerable. The reason for this could be a higher hypercapnic chemosensitivity in men compared to women of the same age group. This is because progesterone seems to enhance the hypercapnic chemosensitivity. Because the levels of progesterone in women decreases through the hormonal changes during menopause, it is possible that the hypercapnic chemosensitivity also decreases in postmenopausal women. At the moment, there are no available systematic studies of the HCVR from postmenopausal women and men of the same age. The aim of this study was to test the hypothesis that the hypercapnic chemosensitivity is significantly lower in postmenopausal women over 60 years than in men of the same age group. The HCVR was measured by Read’s rebreathing method whereby a hyperoxic gas mixture with an initial share of 7% CO2 was rebreathed in a closed system. This is an internationally accepted standard method. The hypercapnic respiratory response is here measured as the change in the minute volume per increase in end-tidal CO2 partial pressure (∆AMV/∆ETCO2). After measuring the hypercapnic chemosensitivity for 51 men and 49 women who are over 60 years old, the results obtained where 1,22 ± 0,65 l/min/mmHg for women and 2,15 ± 1,07 l/min/mmHg for men. Definitely, the nearly 50% decrease in women as compared to men underscores the basis of this hypothesis. Based on the results of this study, one can argue that the decreased hypercapnic chemosensitivity in postmenopausal women is a kind of protection against the occurrence of Cheyne-Stokes respiration. This could be a therapeutic approach in the form of medicamentous reduction of the hypercapnic chemosensitivity in men.
