Abstract

The number of patients receiving palliative care has risen in recent years. Comprehensive care that takes physical, psychological, social and spiritual aspects into account is important for palliative patients. It is not known whether patients who are treated or die in a palliative care unit differ in aspects of care from patients in specialised palliative home care. The aim of the study was to understand which aspects of care are important for patients, relatives and healthcare professionals in hospital and outpatient settings and how these aspects differ in the two settings. A qualitative design based on semi-structured interviews that were analysed using qualitative content analysis was used for this purpose. The study took place in Hesse. Data was collected through interviews with patients, relatives and staff on the palliative care ward at Marburg University Hospital. These interviews were compared with data from the ELSAH study, which was conducted in the setting of specialised outpatient palliative care. There were several aspects of care that were relevant in both settings: Symptom control, physical autonomy, psychological well-being, sense of security, self-determination, care planning, patient-centred care and spiritual rituals. In addition, the immediate availability of medical professionals for symptom control, greater relief for relatives and faster availability of psychological and spiritual care proved to be particularly important in the inpatient setting. Knowledge of these aspects can contribute to the improvement of comprehensive care by taking the identified issues into account in the cooperation between inpatient and outpatient palliative care and expanding networks in the outpatient setting accordingly.
