Eine retrospektive Analyse möglicher prognostischer Faktoren für Patienten mit nicht-kleinzelligem Lungenkarzinom im fernmetastasierten Tumorstadium

Abstract

Introduction Metastasized non-small cell lung cancer (NSCLC) has one of the worst prognoses of all solid tumours. Yet, a minority of patients with stage IV NSCLC shows long-term survival. However, little is known about specific characteristics constituting this specific patient group. Methods Our study compared three patient cohorts with stage IV NSCLC (diagnosed 2008-2012): Two cohorts contained patients with a survival >5 years and 2-5 yrs, respectively. The third cohort matched an equal number of deceased patients with a survival <2 yrs, each one diagnosed subsequent to a patient of one of the former cohorts. The following data (derived from our hospital lung cancer registry/information system) were collected and compared: extended patient features, laboratory findings throughout the continuum, tumour details with molecular pathology, therapeutic data including response to treatment, and follow-up data. Results In 2008-2012, 1,221 stage IV NSCLC patients were diagnosed in our hospital. Out of these, 17 and 87 patients were detected with a survival > 5 and 2-5 yrs, respectively, and matched as described. Patient figures may increase since patients with a lost-to-follow-up are currently further enquired. Statistical analyses revealed differences among the cohorts regarding age, gender, performance status, comorbidities, laboratory values, histology, molecular pathology, treatment, and response to treatment. Conclusion Based on our findings, long term survivors in stage IV NSCLC seem to share certain clinical characteristics discriminating them from those with poorer prognosis. However, extended molecular profiling may better describe and predict longterm survivorship.
