Abstract

Nephroureterolithiasis is one of the most frequent diseases requiring treatment by a urologist. The disease is influenced by a wide variety of factors, e.g. diet, genetics and climate. Treatment is continuously changing due to technical progress, miniaturization of instruments, as well as socio-economic aspects. The purpose of this retrospective, population-based work was to analyse nephroureterolithiasis and its treatment in Germany. After defining relevant ICD-10 codes (N20.0; N20.1; N20.2; N20.9 and N23) data was requested from the Federal Statistical Office, including associated OPS-Codes. Age- and gender-distribution, distribution by state, mode of discharge and associated procedures to facilitate urine passage. Prevalence is stable for Germany, with regional foci in North Rhine-Westfalia and Rhineland-Palatinate. Therapy has seen a shift from extracorporal shockwave lithotripsy to endoscopic treatments i.e. ureterorenoscopy (URS) and percutaneous nephrolithotomy (PNL). The increase in usage of URS seems to be stagnating at a stable plateau. Use of PNL, while not as frequent, is seeing a continued increase. Combined kidney and ureter stones are increasing in prevalence, while isolated kidney- or ureter stones remain stable. Distribution by gender shows a clear dominance with males outnumbering females about 2:1 and ureter stone is the most frequent diagnosis. One exception in gender distribution is renal colic, here females outnumber males. Age distribution shows a small but growing percentage of elderly patients (>80 years). Length of stay is declining in all groups above the age of 20 years and procedures facilitating a quick discharge are preferred.
