Abstract

Patients with Parkinson‘s disease develop individual healthcare needs as their disease progresses. Even before the COVID-19 pandemic, access to healthcare services was unevenely distributed depending on certain structural and personal determinants. Due to limited resources during the pandemic, Parkinson´s patients were at particular risk of an inadequate care. The aim of this study was to find out which factors influencing access to healthcare services affect satisfaction with healthcare and the perception of inadequate care among patients with Parkinson‘s disease during the COVID-19 pandemic in Germany. To this end, we conducted an anonymous online survey among members of the Deutschen Parkinson Vereinigung e.V. (German Parkinson‘s Association). The questionnaire was created as part of the international iCARE-PD project and was available to participants in Germany from November 2020 to January 2021. A generalized linear model was used to identify significant predictors that led to higher odds of patients reporting feeling inadequately cared für during the COVID-19 pandemic. In a further step, stepwise regression was used to condense the essential determinants into a reduced model. The evaluation of the 551 returned questionnaires showed that satisfaction with Parkinson´s-specific healthcare decreased significantly compared to the period before the COVID-19 pandemic. At the level of individual determinants, a low level of education, financial difficulties, female gender, and the perceived expertise on the part of the treating physicians contributed particularly to higher odds of perceiving inadequate care. At a structural level, the density of neurologists in the place of residence and a low level of trust in the use of telemedicine services were identified as important determinants. Patients who already had difficulties accessing specialized healthcare services and had reduced availability of such services before the pandemic were also at risk. The use of telemedicine can help facilitate access to healthcare services, while taking into account the inherent risks of promoting inaqualities. Although the proportion of users increased in our population, it remained at a relatively low level during the first months of the COVID-19 pandemic. These findings can help to derive measures to protect particularly vulnerable groups from undersupply, fundamentally improve their access and satisfaction with healthcare and make health systems more resilient to crises.
