Abstract

Summary The present study demonstrates that targeted postoperative antibiotic therapy for appendicitis should be optimized by a more precise intraoperative assessment of the degree of inflammation. Currently, in approximately 80% of cases, it is possible to correctly differentiate between uncomplicated and complicated appendicitis based on laparoscopic findings. Nevertheless, unnecessary antibiotic prescriptions frequently occur in cases of uncomplicated appendicitis, although such treatment is indicated only in complicated cases. To limit the further development of antibiotic resistance, a reduction in these unnecessary prescriptions is desirable. Standardized criteria that enable pediatric surgeons to make an intraoperative assessment and accordingly determine postoperative management would be helpful. The criteria established by the National Surgical Quality Improvement Program (NSQIP) Pediatric from the USA could also be implemented in German clinics. Once this assessment is made, it is crucial to tailor postoperative management accordingly. The choice of antibiotics in complicated appendicitis should be standardized within the clinic, taking into account the local resistance patterns and the individual clinical course of each patient. At the Department of Pediatric Surgery of the University Hospital Gießen and Marburg, Marburg site, a guideline with recommendations for antibiotic therapy in uncomplicated and complicated cases of appendicitis has already been developed. Surgeons’ understanding and confidence should be reinforced to omit postoperative antibiotic therapy in cases of uncomplicated appendicitis. The results underscore the necessity of standardized assessment systems and their integration into clinical guidelines to address both individual therapeutic success and the global challenge of antibiotic resistance.
