Pericapsular Nerve Group Block zur postoperativen Analgesie nach elektiver Hüft-Total Endoprothese Implantation

Abstract

Introduction: The total hip arthroplasty (THA) is frequently associated with increased opioid use in the postoperative period. The pericapsular nerve group (PENG) block may offer an opioid-sparing approach to pain management as a regional anesthesia technique. Previous studies have shown varying results regarding the need for morphine equivalents following the use of the PENG block. The aim of the present study was to evaluate, as the primary outcome, the morphine equivalent consumption and, as the secondary outcome, the maximum NRS Score within 48 hours after elective total hip arthroplasty. Materials and Methods: A total of 309 patients who underwent elective total hip arthroplasty were included in this retrospective study. 141 patients received a PENG block (intervention group), and 168 patients did not (control group). The intervention group received a PENG block with 40 ml of 0.2% ropivacaine after general anesthesia, while the control group received only general anesthesia without any regional block. Maximum pain intensity was recorded using the Numerical Rating Scale (NRS), and total morphine equivalent (MEQ) consumption in milligrams was measured over a 48-hour postoperative period. Results: The results showed that the average morphine equivalent consumption in the PENG group was not statistically lower than in the control group. However, the comparison of pain intensity (NRS) revealed statistically significantly lower pain levels on the day of surgery and 24 hours postoperatively in the group that received the PENG block. Conclusion: This study indicates that the use of the PENG block in the investigated cohort does not have a significant impact on postoperative morphine equivalent consumption. However, it remains unclear whether this technique may demonstrate a clear benefit in other patient populations, different surgical procedures, or in combination with additional regional anesthesia techniques. Several prospective, randomized trials are required to address this question.
