Abstract

Psychopathy in medical students and doctors strongly effects medical school success, the doctor-patient relationship, and the quality of collaboration in medical teams. However in Germany, this sensitive topic is rarely investigated. The present study compared standardized behavioral observations of medical students for psychopathic tendencies (PT) to self-assessment through a questionnaire. It further was investigated if PT students, after self-assertiveness training (SST), increased their empathy in a patient conversation. Medical school students (n = 343) at the Philipps-University Marburg were assessed in the 1st and 3rd semester. An 8-minute standardized behavioral observation of conversation with a patient-actor was made. A coding scheme which both specificity and sensitivity as well as PT criteria was developed. The study revealed PT in medical students (17.2 %, N = 59). SST had only a limited impact and empathic behavior could not be learned. Importantly, a sensitivity of 0.49 and a specificity of 0.93 resulted from the coding scheme allowed a high empathy categorization. Through the selectivity calculation, a reduction to 3 PT criteria was possible, namely manipulation, quick-tempered impulsivity, and dominance. This study showed medical student PT and the need for “Codes of business/medical Ethics“. Future studies should determine if these PT criteria can be developed to help select medical school applicants.
