Abstract

The aim of this dissertation was, on the one hand, to understand the evidence-based risk for other cancers in women with breast cancer compared to a healthy collective and, on the other hand, to analyze the treatment adherence of women taking tamoxifen (TAM) or aromatase inhibitors (AIs) in Germany and to investigate possible determinants of non-persistence. The work was based on two studies published in PubMed-listed journals. The first study was based on data from the IMS Disease Analyzer database and the second from the Longitudinal Prescription (LRx) database. The first article examined the association between breast cancer and the incidence of other cancer diagnoses and was published on July 12, 2022 in Breast Cancer Research and Treatment. Methodologically, it is a retrospective cohort study with 42,248 patients. The strongest association was observed for cancer of the respiratory tract, followed by cancer of the female genital organs and cancer of the lymphatic and hematological tissue. In summary, the results of this first study show that breast cancer patients have an increased risk of developing another type of cancer compared to healthy women. The second article showed that persistence rates for both TAM and AI treatment were low across all age groups and identified several factors (e.g. specialist, younger age and type of endocrine therapy) that were associated with an increased risk of non-persistence. This article was published in the Journal of Cancer Research and Clinical Oncology on September 23, 2022. Methodologically, this is a retrospective cohort study of 284,383 patients. Many women survive a diagnosis of breast cancer. Knowing their risk of developing another primary cancer is important not only in terms of potential side effects of their cancer therapy, but also in terms of the possibility of a common etiology with other cancers. This higher risk of a second cancer in women who were diagnosed with primary breast cancer as their first malignant disease underlines the importance of preventive measures to reduce the incidence and early detection measures for the diagnosis of second cancers. The second study underlines the need for good care for patients undergoing antihormonal therapy, for example as part of a DMP. However, the results of the two studies must be viewed with caution, as the databases have several limitations, the coding quality of the general practitioners is inaccurate and no causal effects can be determined due to the study design.
