Abstract

Background: Patients suffering from sleep disordered breathing have a high rate of morbidity and mortality caused by accompanying cardio-vascular diseases. Sleep disordered breathing is diagnosed in 30 to 40% of patients with coronary heart disease. The aim of this study was to examine the relationship of nocturnal myocardial ischemia and disordered breathing in patients with coronary heart disease. Methods: We used for this observational study a 12-lead long term Holter recording(WelchAllyn CardioPerfect Holter-ECG) and a synchronized sleep apnea screening device (SOMNOcheck WM 94100, Weinmann) to examine nocturnal breathing disorders and depressions in the ST-segment. Results: 19 patients with coronary heart disease were included. An apnea-hypopneaindex(AHI) of more than 20 was observed in 7 patients (36,8%) (range 21 to 58), an AHI of 10 to 20 was observed in 3 subjects (range 11 to 19). 13 episodes of nocturnal depressions in the ST-segment were found in 8 subjects. 11 depressions occurred after a preceding desaturation, 2 without. Conclusion: The supposed relation between nocturnal desaturation and myocardial ischemia could not be verified. Other factors such as a change in the sympathovagal balance could contribute to nocturnal myocardial ischemia.
