Abstract
Introduction: Daytime sleepiness is a symptom of obstructive sleep apnea.
Recent studies suggest that sleep fragmentation and arousals may not be the
main causes of daytime sleepiness. Several studies have reported an association
between obstructive sleep apnea and inflammatory markers. This study aimed to
investigate whether increased levels of interleukin-6, C-reactive protein and
tumor necrosis factor alpha are associated with daytime sleepiness in obstructive
sleep apnea patients.
Methods: A total of 103 untreated obstructive sleep apnea patients were
assessed for daytime sleepiness using the Epworth Sleepiness Scale. All
patients underwent polygraphy or polysomnography and a fasting blood sampling
to determine interleukin-6, C-reactive protein and tumor necrosis factor alpha. 47
patients also completed a vigilance test (VigiMar).
Results: The correlation between Epworth Sleepiness Scale and interleukin-6
was not statistically significant but showed a trend (p= 0.063, r= 0.184). Similarly
for Epworth Sleepiness Scale and C-reactive protein (p= 0.066; r= 0.192) and for
Epworth Sleepiness Scale and tumor necrosis factor alpha (p= -0.068; r= 0.497).
No significant correlations were found between the VigiMar test results and the
inflammatory markers. A highly significant correlation with a strong effect size
was found between the apnea-hypopnea index and C-reactive protein (p <0.001;
r= 0.457). Apnea-hypopnea index and interleukin-6 also correlated significantly,
but with a smaller effect (p= 0.047; r= 0.148). Between the obstructive sleep
apnea severity groups (AHI 5-<15; 15-<30; >30), there were significant
differences in C-reactive protein values, most prominently between the mild
obstructive sleep apnea group and the others (p ≤0.001). After adjusting for body
mass index, the level of significance of the correlations between apnea-hypopnea
index and interleukin-6 was reduced below the significance level. Between
apnea-hypopnea index and C-reactive protein the correlations was still significant
(p= 0,004).
Conclusion: No significant association was found between daytime sleepiness
(ESS, VigiMar) and interleukin-6, C-reactive protein or tumor necrosis factor
alpha. A significant correlation exists between apnea-hypopnea index and both

C-reactive protein and interleukin-6. The significant correlation between apneahypopnea index and C-reactive protein remained significant after adjusting for
body mass index. The relationship between apnea-hypopnea index and
interleukin-6 is likely explained by the strong association with body mass index,
which showed the highest correlation with the inflammatory markers.
