Abstract
The aim of this study is to compare the volume measured in the three-dimensional (3D) breast photography with the volume calculated in the mammography and the volume of the resected breast specimen measured by water displacement. If correct results for the three-dimensional breast photography can be evaluated, this method would be more convenient, faster and more economical than all the previous methods that have been described in the literature for calculating the breast volume.
For the 3D-breast photography five pictures were taken utilizing two cameras, while the patient was presented at a 90°, 45° angle left and right side, and frontal position. The distance between the two cameras, as well as the distances between the patient and the camera, the patient and the reference sheets was given. In addition to that the breast volume was calculated based on six calculation methods, described by Kalbhen et al. using mammograms. The volume of the resected breast specimen measured by water displacement served as a reference parameter. For the internal control of this reference parameter the resected breast specimen was also weighed.
By means of this 3D-breast photography the breast is presented in its three dimensions, which leads to the hypothesis that the 3D-volume of the breast may be calculated using those pictures.
Concerning the question regarding the comparison between the breast volume calculated by use of mammography and the breast volume quantified through 3D-breast photography, a difference of 13% on average was shown. The percental variance of the comparison between the breast volume calculated based on the mammograms and the volume of the resected breast specimen, measured by water displacement, was 73% on average. The last question refers to the comparison between the measured breast volume of the resected breast specimen through water displacement and the breast volume quantified through 3D-breast photography revealing a great difference of 67% on average. The correlation coefficients of the comparison between the calculated breast volume by mammograms and the volume of the resected breast specimen quantified by water displacement, as well as the correlation coefficients of the comparison between 3D-breast photography and the volume of the resected breast specimen utilizing water displacement, resulted in 0.86-0.93 or 0.86-0.92. Though this high correlation was also cross-checked through the graphical presentation of Bland-Altman-diagrams showing an excessive divergence. As a result neither the breast volume calculated through mammograms, nor the 3D-breast photography are beneficial at this point of time to determine the “true” breast volume measured by use of the Archimedean principle. Therefore the hypothesis to calculate the breast volume based on five pictures utilizing two cameras is rejected. Through the evaluation process possibilities for improvement were discussed, such as an increased number of pictures and/or a fixed position of the patient as well as additional pictures from different angles. These modifications of the study design may lead to a more precise calculation of the breast volume. Therefore this hypothesis should be verified in a further study.
