Abstract
Prevalence rates indicate high levels of distress and a significant reduction of mental health in medical students. A large number of international studies addresses the effectiveness of stress prevention programs in medical schools, whereas systematic research is lacking for Germany. Taking this into account, the present doctoral thesis aims at developing and evaluating a stress prevention training with the objective to overcome some limitations of former studies by using rigorous scientific methods.
The first publication presents the conceptual design of the specifically developed stress prevention training “MediMind – mindfulness-based stress prevention training for medical students” and explains the methodological considerations of the evaluation. In a randomized controlled trial three study conditions were to be compared: experimental treatment (MediMind), standard treatment (Autogenic Training) and a control group. Distress, coping and psychological morbidity as outcome variables were to be examined in order to establish clinical efficacy, involving multiple assessment time points.
In the second publication the results of the evaluation are presented and discussed critically. Based on a repeated measures analysis of variance design, no preventive effect of the intervention groups (MediMind and Autogenic Training) could be revealed. The interpretation of the results has to take the high dropout rate of the assessments into account that contrasts the stable participation in the trainings. A systematic dropout effect indicates that students suffering more often from psychological symptoms at baseline assessment did not participate in the follow-up assessments. Consequently, the extent to which the results can be generalized is limited.
In summary, the present thesis provides important indications to the necessity and acceptance of stress prevention approaches in German medical schools. Although the relative efficacy of different intervention programs remains unclear, it could be shown that the students used the skills provided by MediMind over an extended period of time. This observation should be considered when setting up future training programs.
